Make a Referral
To enquire about an assessment or request an appointment, please complete the referral form below or contact our office on (02) 9415 3116.
Our administrative team will review your enquiry and contact you to discuss availability, fees, and any information required to progress the referral.
Assessment Locations
Assessments may be conducted:
- At our offices
- Via video conference
- Via Audio Visual Link (AVL)
- Within correctional centres and youth detention facilities
- At other agreed locations where appropriate
Custody Matters
We regularly conduct assessments for individuals in custody throughout New South Wales and can assist with arranging AVL and correctional centre bookings.

Booking Form
Our office staff will also make telephone bookings on 02 9415 3116 for office or in custody appointments. The date, approximate time and name of the consultant who will be conducting the assessment, will be given. A Booking Confirmation form will be forwarded containing our various contact details, the date time and place of the appointment and a map is attached with parking and transport information for office appointments.
We attend all metropolitan and country gaols and juvenile detention centres in NSW to conduct comprehensive face to face assessments. Video link assessments will also be accepted if necessary. In addition, we regularly conduct assessments in other locations such as lawyers’ rooms or other agreed suitable venues.
